Consultation forms and patch tests

Create a form

1. Go to WearBeauty, Forms.
2. Start from one of the seven templates (general health, hair colour, lash and brow, waxing, aesthetics medical history, massage and spa, spray tan) or a blank form.
3. Add questions: short or long answers, yes or no, pick one, pick any, dates, tick to agree and a signature.
4. Choose which services the form is for, or every appointment.
5. Choose how often to ask again, in months, and whether to ask again when you change the questions.

How clients fill it in

• The form is sent automatically when a booking is confirmed.
• You can send it from the client’s page at any time.
• Or click Fill in now on the appointment to complete it on a tablet in the salon.

Answers you flag become a pinned note on the client and show in the calendar. Answers can be marked clinical so only clinical staff see them.

Record a patch test

1. Open the client and add a patch test with the service, product, date and time.
2. Record the result: passed, reaction (do not treat) or waiting for result.

A patch test must be at least 48 hours before treatment and is valid for six months. Services marked Needs a patch test cannot be booked online until a passed test is on file.
